Categories must be activated each quarter or earnings drop to 1%. No annual fee, but 3% foreign transaction fee. Sign-up bonus: $250 ($25,000 points) after $1,000 spend in 90 days. Cash+ also earns 5% on prepaid air, hotel, and car reservations booked through the U.S. Bank Travel Center; CardSavvy doesn't model travel-portal earn rates yet, so direct travel spend is shown at 1%.
